Intravenous cefotaxime 1 gm iv q8h was given from the referral hospital and stopped on day 1 of hospitalization. Amoxicillin-clavulanate was given on day 20 and stepped up to piperacillin-tazobactam on day 24 of hospitalization for nosocomial onset of fever of unknown source. The dotted horizontal line denoted the detection limit of VRE in fecal samples by broth enrichment ~ 200 cfu/g (2.3 log10 cfu/g).
